Microsporum canis (М. canis) – зоофильный дерматофит, возбудитель дерматофитий человека и животных. Микоз волосистой части головы (ВЧГ), обусловленный М. canis, сопровождающийся вовлечением в процесс значительной площади кожных покровов, редко развивается у взрослых из-за физиологических особенностей, обусловленных низким pH и высокой концентрацией полиненасыщенных жирных кислот в себуме. Эпидемиологические данные о частоте микоза среди взрослых в разных странах варьируют от 1,5 до 44%. Позднее обращение, ошибки в диагностике и применение топических кортикостероидов способствуют диссеминации процесса. В связи с повсеместным ростом числа случаев заражения микроспорией у взрослых ситуация диктует необходимость повышения информированности врачебного сообщества.</p></sec><sec><title>Цель</title><p>Цель. Провести анализ клинических и лабораторных особенностей инфекции М. canis у взрослой пациентки.</p></sec><sec><title>Результаты</title><p>Результаты. В статье приводится описание случая микоза ВЧГ с диффузным поражением гладкой кожи, вызванным М. canis, у взрослой пациентки. Эффективное лечение было отсрочено на год по причине неправильной клинико-лабораторной диагностики заболевания. В ГБУЗ «Московский центр дерматовенерологии и косметологии» при лабораторном исследовании патологического материала получен рост культуры М. canis. При прямой микроскопии выявлено расположение спор по типу ectothrix. Лечение системным итраконазолом по непрерывной схеме дало положительный результат в течение шести недель.</p></sec><sec><title>Выводы</title><p>Выводы. Микоз ВЧГ с обширным вовлечением в процесс кожных покровов туловища и конечностей, вызванный M. canis, является клинической редкостью среди взрослых в большинстве регионов, что подчеркивает сложность диагностики распространенной дерматофитной инфекции и в данном клиническом примере. В случаях, проявляющихся обширной эритемой с шелушением на коже головы и туловища, следует исключить грибковую инфекцию. Требуется тщательный сбор эпидемиологического анамнеза, включая сведения о миграции, контактах с животными и физикальный осмотр на предмет обломанных волос, люминесцентная диагностика и микологические исследования.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Microsporum canis (M. canis) is a zoophilic dermatophyte, causative agent of human and animal dermatophytosis. Scalp mycosis caused by M. canis, accompanied by involvement of a large area of the skin, rarely develops in adults due to physiological peculiarities caused by low pH and high concentration of polyunsaturated fatty acids in sebum. Epidemiological data on the incidence of mycosis in adults vary from 1.5 to 44% in different countries. Late treatment, diagnostic errors and the use of topical corticosteroids contribute to the dissemination of the process. Due to the widespread increase in the number of adult cases of microsporidia infection, the situation necessitates the need to increase awareness of the medical community.</p></sec><sec><title>Aim</title><p>Aim. To analyse the clinical and laboratory features of M. canis infection in an adult patient.</p></sec><sec><title>Results</title><p>Results. This article describes a case of scalp mycosis with diffuse skin lesions caused by M. canis in an adult patient. Effective treatment was delayed for a year due to incorrect clinical and laboratory diagnosis. In Moscow Scientific and Practical Center of Dermatovenereology and Cosmetology» during the laboratory examination ial the growth of M. canis culture was obtained. Direct microscopy revealed the location of spores as an ectothrix. Treatment with systemic itraconazole according to a continuous scheme gave a positive result within six weeks.</p></sec><sec><title>Conclusion</title><p>Conclusion. Scalp mycosis with extensive involvement of the trunk and extremities caused by M. canis is clinically uncommon in adults in most regions, emphasising the difficulty in diagnosing widespread dermatophyte infection in this clinical example. In cases presenting with extensive erythema with scaling on the scalp and trunk, fungal infection should be excluded.
