Атопический дерматит (АтД) – хроническое воспалительное заболевание кожи, существенно влияющее на качество жизни пациентов. Появление таргетной биологической терапии дупилумабом стало важным этапом в лечении среднетяжелого и тяжелого АтД, что подтверждено клиническими исследованиями и данными реальной клинической практики. Цель. Изучить динамику качества жизни у пациентов с АтД, получающих дупилумаб, в реальной клинической практике.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен обзор современной литературы, включающей результаты клинических исследований и регистровых данных, а также анализ собственной выборки из 38 пациентов с АтД, получавших терапию дупилумабом. Оценивалась динамика клинических индексов (EASI, SCORAD, VAS) и показатели качества жизни (DLQI – у взрослых и CDLQI – у детей).</p></sec><sec><title>Результаты</title><p>Результаты. Возраст пациентов варьировал от 6 до 47 лет (средний – 20 лет), мужчины составили 65,8%. Продолжительность терапии составляла от 2 до 5 лет (в среднем – 3,4 года). На фоне лечения средний индекс EASI снизился с 39,0 до 5,2 (p &lt; 0,0001), SCORAD – с 58,7 до 11,2 (p &lt; 0,0001), VAS – с 7,3 до 2,2 (p &lt; 0,0001). Показатели качества жизни улучшились: DLQI снизился с 25,1 до 2,3, CDLQI – с 21,1 до 2,4 (оба p &lt; 0,00001). Полученные данные согласуются с международными исследованиями (TREATgermany, BioDay, RELIEVE-AD).</p></sec><sec><title>Выводы</title><p>Выводы. Дупилумаб демонстрирует высокую эффективность, значимое снижение клинических проявлений и улучшение качества жизни у взрослых и детей с АтД при длительном применении в реальной клинической практике. Сочетание данных рандомизированных исследований и реальной практики подтверждает, что дупилумаб является препаратом выбора и может рассматриваться как оптимальная долгосрочная терапевтическая стратегия для пациентов с АтД.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Atopic dermatitis (AD) is a chronic inflammatory skin disease that significantly impairs patients’ quality of life. The advent of targeted biologic therapy with dupilumab has become an important milestone in the treatment of moderate-to-severe AD, as confirmed by clinical trials and real-world data.</p></sec><sec><title>Aim</title><p>Aim. To study the dynamics of quality of life in patients with AD receiving dupilumab in real clinical practice.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A review of current literature, including results from clinical trials and registry data, was conducted, along with an analysis of our own cohort of 38 patients with AD treated with dupilumab. The dynamics of clinical indices (EASI, SCORAD, VAS) and quality-of-life measures (DLQI in adults and CDLQI in children) were evaluated.</p></sec><sec><title>Results</title><p>Results. Patients’ ages ranged from 6 to 47 years (mean – 20 years), with males accounting for 65.8%. The duration of therapy ranged from 2 to 5 years (mean – 3.4 years). During treatment, the mean EASI score decreased from 39.0 to 5.2 (p &lt; 0.0001), SCORAD from 58.7 to 11.2 (p &lt; 0.0001), and VAS from 7.3 to 2.2 (p &lt; 0.0001). Quality-of-life measures improved markedly: DLQI decreased from 25.1 to 2.3, and CDLQI from 21.1 to 2.4 (both p &lt; 0.00001). These findings are consistent with international studies such as TREATgermany, BioDay, and RELIEVE-AD.</p></sec><sec><title>Conclusion</title><p>Conclusion. Dupilumab demonstrates high efficacy, significant reduction in clinical manifestations, and substantial improvement in quality of life in both adults and children with AD during long-term real-world use.
