В России, где заболеваемость и смертность превышают средние общемировые показатели, до сих пор не существует национальных программ по скринингу, ранней диагностике и профилактике РЖ. Цель обзора – проанализировать современные литературные данные о заболеваемости, смертности и методах скрининга РЖ в различных странах мира и в России. Средний стандартизированный по возрасту показатель (ASR) заболеваемости РЖ в мире составляет 9,2 на 100 000 населения, ASR смертности – 6,1 на 100 000, а пятилетняя выживаемость за 2000–2014 гг. варьирует от 20 до 40%. В России заболеваемость и смертность от РЖ равны 13,7 на 100 000 и 9,2 на 100 000 населения соответственно. Неблагополучные эпидемиологические показатели РЖ отмечаются в Республике Тыва: ASR заболеваемости в этом регионе составляет 24,56 на 100 000 населения, а ASR смертности – 18,75 на 100 000 населения. Программы популяционного скрининга РЖ, осуществляемые всего в двух странах мира (Южной Корее и Японии), существенно повысили пятилетнюю выживаемость при РЖ в этих странах – до 68,9 и 60,3% соответственно, тогда как в США и России этот показатель составляет 33,1 и 21% соответственно. Ведущим методом популяционного и оппортунистического скрининга РЖ в мире и России является эндоскопия у лиц в возрасте старше 50 лет. Для оппортунистического и популяционного скрининга рекомендуется проведение эзофагогастроскопии у лиц старше 50 лет 1 раз в 3 года, а также использование серологических тестов с определением уровня пепсиногенов I и II и IgG к Helicobacter pylori. Высокие показатели заболеваемости и смертности от РЖ в России и низкий уровень пятилетней выживаемости определяют актуальность разработки и внедрения национальной стратегии популяционного скрининга РЖ.</p></abstract><trans-abstract xml:lang="en"><p>Gastric cancer (GC) is a major social issue that is particularly acute in East Asia, Eastern Europe, and South America. Russia, where morbidity and mortality rates are higher than the global averages, still has no national GC screening, early detection and prevention programs. The aim of this review is to analyze current literature data on the incidence, mortality, and screening methods of gastric cancer (GC) in different countries of the world and in Russia. The average age-standardized rate (ASR) of GC incidence worldwide is 9.2 per 100,000 population, ASR of mortality is 6.1 per 100,000, and five-year survival for 2000–2014 varies from 20% to 40%. In Russia, the incidence and mortality rates of GC are 13.7 per 100,000 and 9.2 per 100,000, respectively. Unfavorable epidemiological indicators of GC are observed in Republic of Tyva: the ASR of incidence in this region is 24.56 per 100,000 population, and the ASR of mortality is 18.75 per 100,000 population. Population screening programs for GC, implemented in only two countries in the world (South Korea and Japan), have significantly increased the five-year survival rate for GC in these countries to 68.9% and 60.3%, respectively, while in the USA and Russia this figure is 33.1% and 21%, respectively. The leading method of population and opportunistic screening of GC in the world and in Russia is endoscopy in individuals over 50 years of age. For opportunistic and population screening, it is recommended to use esophagogastroscopy in individuals over 50 years of age once every 3 years and serological tests to determine the level of pepsinogens I and II, and IgG to Helicobacter pylori.
