Ревматоидный артрит (РА) – это заболевание, для которого характерен эпидемиологический половой диморфизм (страдают преимущественно женщины). У многих из них дебют заболевания приходится на возраст, близкий к наступлению менопаузы, поэтому актуальным является анализ взаимосвязи между РА и дефицитом эстрогенов.</p></sec><sec><title>Цель</title><p>Цель. Изучить клинические особенности РА у женщин в зависимости от возраста наступления менопаузы.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Было проведено ретроспективное исследование 246 женщин в постменопаузе с установленным диагнозом РА, находящихся в постменопаузе. Пациенты были разделены на подгруппы в зависимости от возраста наступления менопаузы на раннюю (до 45 лет, n = 71) и своевременную менопаузу (≥45 лет, n = 175), между подгруппами было проведено сравнение по количественным и качественным показателям. В подгруппе с ранней менопаузой было проведено разделение в зависимости от естественной (n = 46) или хирургической причины (n = 25) ее наступления и последующим проведением межгруппового сравнения.</p></sec><sec><title>Результаты</title><p>Результаты. Женщины с ранней менопаузой по основным клиническим и лабораторным показателям, характеризующим РА, были сопоставимы с группой со своевременным наступлением менопаузы. Женщины с естественным ранним наступлением менопаузы, в сравнении с ранней хирургической менопаузой, характеризовались значимо более высоким уровнем С-реактивного белка (6,45 [3,35; 22,33] против 3,20 [1,70; 10,90] мг/л; p = 0,03) и СОЭ (28,50 [16,50; 58,00] против 16,00 [11,00; 29,00] мм/ч; p = 0,021), а также трендами к более высокой степени активности по DAS28 (p = 0,06), большему числу припухших суставов (4,50 [2,25; 8,00] против 3,00 [1,00; 6,00]; p = 0,089) и большей частоте эрозивного артрита (78,3 против 56,0%; p = 0,077). С другой стороны, при ранней хирургической менопаузе отмечалось более высокая частота наличия сердечно-сосудистых заболеваний и артериальной гипертензии (72,0 против 34,8%; p = 0,003), а также более высокий уровень глюкозы натощак (5,49 [4,88; 5,79] против 5,00 [4,79; 5,40] ммоль/л; p = 0,03) в сравнении с женщинами с ранним естественным наступлением менопаузы.</p></sec><sec><title>Выводы</title><p>Выводы. Возраст наступления менопаузы в нашей выборке не оказывал существенного влияния на особенности клинического течения РА, однако женщины с ранней естественной менопаузой имели более высокие показатели, отражающие воспалительную активность РА, а женщины с ранней хирургической менопаузой чаще страдали метаболическими нарушениями.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Rheumatoid arthritis (RA) is a disease characterized by epidemiological sexual dimorphism (affecting predominantly women). For many of women, the onset of the disease occurs around menopause, making it important to analyze the relationship between RA and estrogen deficiency.</p></sec><sec><title>Aim</title><p>Aim. To study the clinical features of RA in women depending on the age of menopause.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospective study of 246 postmenopausal women with an established diagnosis of RA in postmenopause was conducted. Patients were divided into subgroups depending on the age of menopause: early (before 45 years, n = 71) and timely menopause (≥45 years, n = 175), a comparison of quantitative and qualitative indicators was made between the subgroups. In the subgroup with early menopause, a division was made depending on the natural (n = 46) or surgical cause (n = 25) of its onset, followed by an intergroup comparison.</p></sec><sec><title>Results</title><p>Results. Women with early menopause were comparable with the group with timely onset of menopause in terms of the main clinical and laboratory parameters characterizing RA. Women with natural early menopause, compared with early surgical menopause, were characterized by significantly higher levels of C-reactive protein (6.45 [3.35; 22.33] mg/L vs. 3.20 [1.70; 10.90] mg/L; p = 0.03) and ESR (28.50 [16.50; 58.00] mm/h vs. 16.00 [11.00; 29.00] mm/h; p = 0.021), as well as trends toward higher DAS28 activity (p = 0.06), more swollen joints (4.50 [2.25; 8.00] vs. 3.00 [1.00; 6.00]; p = 0.089), and a higher incidence of erosive arthritis (78.3% vs. 56.0%; p = 0.077). On the other hand, early surgical menopause was associated with a higher incidence of cardiovascular disease and hypertension (72.0% versus 34.8%; p = 0.003), as well as higher fasting glucose levels (5.49 [4.88; 5.79] mmol/l versus 5.00 [4.79; 5.40] mmol/l; p = 0.03) compared to women with early natural menopause.</p></sec><sec><title>Conclusions</title><p>Conclusions.
