Хронический гнойный средний отит (ХГСО) это хроническое воспаление полостей среднего уха с образовани­ем стойкой перфорации или ретракции барабанной перепонки различного размера и локализации. В стадии обострения ХГСО является серьезной проблемой здравоохранения, усугубляемой ростом антибиотикорезистентности ключевых пато­генов (S. aureus, P. aeruginosa). Фаготерапия рассматривается как перспективная альтернатива или дополнение к традици­онному лечению.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность и безопасность препарата Секстафаг® (пиобактериофаг поливалентный) в качестве моно­терапии и комбинированной терапии при обострении ХГСО.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен промежуточный анализ данных клинического исследования, в которое вошло 33 паци­ента с ХГСО в стадии обострения, разделенных на 3 группы (по 11 человек): 1) стандартная местная антибактериальная терапия (ципрофлоксацин); 2) монотерапия поливалентным пиобактериофагом, вводимым методом транстимпанального негнетания; 3) комбинированная терапия (пиобактериофаг поливалентный + ципрофлоксацин). Лечение длилось 10 дней.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. К 10-му дню лечения исчезновение отореи достигнуто у 90,9% пациентов в 1-й и 3-й группах и у 72,7% во 2-й группе. Отмечена выраженная клиническая и бактериолитическая эффективность поливалентного пиобактериофага в отношении ключевых возбудителей. Комбинированная терапия показала высокую эффективность (90,9%). Лечение переносилось хорошо, нежелательных явлений не зарегистрировано.</p></sec><sec><title>Заключение</title><p>Заключение. Применение поливалентного пиобактериофага в комплексном лечении обострений ХГСО показало высо­кую эффективность и безопасность, подтверждая его ценность в борьбе с антибиотикорезистентностью и в улучшении исходов заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Chronic suppurative otitis media (CSOM) is a chronic inflammation of the middle ear cavities, resulting in per­sistent perforation or retraction of the eardrum of varying size and location. Exacerbation of CSOM is a major public health problem, which is aggravated by the rise of antibiotic resistance of key pathogens (S. aureus, P. aeruginosa). Phage therapy is considered to be a promising alternative or complement to the traditional treatment.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the efficacy and safety of Sextaphage® (pyobacteriophage polyvalent) as monotherapy and combination ther­apy in exacerbated CSOM.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. An interim data cut was performed of the clinical trial with total 33 patients with exacerbated CSOM included. The patients were divided into 3 groups (11 patients each): 1) standard topical antibacterial therapy (ciprofloxacin); 2) monotherapy by transtympanic delivery of polyvalent pyobacteriophage; 3) combination therapy (pyobacteriophage polyva­lent + ciprofloxacin). Treatment lasted 10 days.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. The clinical resolution of otorrhea was achieved in 90.9% of patients in groups 1 and 3 and in 72.7% of thosed in group 2 by Day 10. The polyvalent pyobacteriophage therapy demonstrated significant clinical and bacteri­olytic efficacy against key pathogens. Combination therapy demonstrated high efficacy (90.9%). Treatment was well tolerated, with no adverse events reported in the study.</p></sec><sec><title>Conclusion</title><p>Conclusion.
