Компенсация утраченной голосовой функции определяет личную и социальную жизнь пациентов после ларингэктомии. Широкое применение находят методы автоматического распознавания и синтеза речи в виде приложений для дополнительной и альтернативной коммуникации. Одной из актуальных задач в клинической практике является оценка эффективности результатов восстановления голосовой функции.</p></sec><sec><title>Цель</title><p>Цель. Оценить эффективность результатов голосо-речевой реабилитации ларингэктомированных пациентов с использованием метода автоматической оценки разборчивости речи.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование были включены 3 группы ларингэктомированных пациентов по 30 человек в зависимости от метода голосо-речевой реабилитации (пищеводный голос, трахеопищеводное шунтирование с голосовым протезированием, голосообразующий аппарат) и 14 пациентов после различных оперативных вмешательств на гортани. Всем пациентам проводилась запись псевдоголоса для дальнейшей оценки фразовой разборчивости с использованием программного модуля автоматической оценки, а также подготовленным и неподготовленным слушателем.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. При сравнительном анализе результатов можно сказать, что система автоматического распознавания демонстрирует минимальный уровень разборчивости, что, по-видимому, связано с семантическим и контекстным распознаванием даже неподготовленным слушателем. Аларингеальная речь демонстрирует худшую распознаваемость по сравнению с органосохраняющими операциями на гортани у неподготовленных слушателей и при автоматическом распознавании. При этом опытные слушатели демонстрируют стабильно высокий уровень распознавания всех видов субститутивной речи.</p></sec><sec><title>Выводы</title><p>Выводы. Объективизация оценки разборчивости субститутивной речи с помощью систем автоматического распознавания позволяет нивелировать семантическое и контекстуальное распознавание при оценке как подготовленным, так и неподготовленным слушателем. Системы автоматического распознавания речи имеют перспективы широкого распространения в восстановительной медицине, в частности в реабилитации пациентов с опухолями головы и шеи. Соотношение распознавания речи и понимания смысла высказывания представляет собой как сложную задачу для разработчиков, так и требует привлечения научной психологии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Lost voice function compensation determines the personal and social life of laryngectomees. Automatic speech recognition and synthesis methods are widely used as apps for additional and alternative communication. One of the urgent tasks in clinical practice is voice restoration effectiveness evaluation.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the effectiveness of voice rehabilitation results in laryngectomized patients using the automatic assessment of speech intelligibility.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. 3 groups of 30 laryngectomized patients depending on the method of voice rehabilitation (esophageal voice, tracheoesophageal voice, electrolarynx), and 14 patients after various surgical interventions on the larynx were included. All patients underwent pseudo-voice recording for further assessment of phrasal intelligibility using the automatic assessment software module, as well as by a trained and untrained listener.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Comparative analysis shows a minimum intelligibility level in automatic recognition, apparently due to semantic and contextual recognition even in untrained listener. Alaryngeal speech demonstrates worse recognition, compared to organ-preserving operations on the larynx, in untrained listeners and in automatic recognition. Experienced listeners demonstrate a consistently high level of recognition of all types of substitute speech.</p></sec><sec><title>Conclusion</title><p>Conclusion. Objectification of the intelligibility assessment of substitute speech using automatic recognition systems allows leveling semantic and contextual recognition during assessment by both trained and untrained listeners.
