Распространенность ожирения в детской популяции приобрела характер эпидемии, что определяет необходимость разработки эффективных методов терапии. Изучение влияния лекарственной терапии на регуляторные пептиды, такие как адипокины и инкретины, представляет значительный научный и практический интерес.</p></sec><sec><title>Цель</title><p>Цель. Оценить динамику уровней адипокинов и инкретинов в период 12-месячной терапии аналогом глюкагоноподобного пептида-1 лираглутидом ожирения у детей.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено одноцентровое проспективное исследование с участием 20 детей в возрасте 12–18 лет с ожирением II–III степени и морбидным ожирением (SDS ИМТ ≥ 2,5). Всем пациентам назначался лираглутид в стандартной дозе 3,0 мг в течение 12 мес. Оценивались антропометрические показатели (масса тела, SDS ИМТ) и уровни адипокинов и инкретинов (лептин, спексин, грелин, обестатин, глюкагоноподобный пептид-1, глюкагон, глюкозозависимый инсулинотропный полипептид, глицентин, оксинтомодулин) до начала терапии, через 1, 4 и 12 мес. лечения.</p></sec><sec><title>Результаты</title><p>Результаты. На фоне терапии лираглутидом отмечено достоверное снижение массы тела и SDS ИМТ (p &lt; 0,05). Выявлено значимое снижение концентрации спексина, глюкагоноподобного пептида-1, обестатина в плазме крови и повышение концентрации глюкозозависимого инсулинотропного полипептида и оксинтомодулина в сыворотке крови. Установлены достоверные корреляционные связи между динамикой SDS ИМТ и уровнями лептина (r = 0,558; p &lt; 0,001), глюкагоноподобного пептида-1 (r = 0,366; p = 0,026) и обестатина (r = -0,364; p = 0,023).</p></sec><sec><title>Выводы</title><p>Выводы. Терапия лираглутидом приводит не только к снижению массы тела, но и к значимым изменениям в профиле адипокинов и инкретинов у детей с ожирением. Динамика изученных биомаркеров, в частности лептина, глюкагоноподобного пептида-1 и обестатина, может быть использована для прогнозирования эффективности терапии аналогами глюкагоноподобного пептида-1. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The prevalence of obesity in the pediatric population has reached epidemic proportions, necessitating the development of effective therapies. Studying the effect of pharmacotherapy on regulatory peptides such as adipokines and incretins is of significant scientific and practical interest.</p></sec><sec><title>Aim</title><p>Aim. To assess the dynamics of adipokine and incretin levels during 12-month therapy with the glucagon-like peptide-1 (GLP-1) receptor agonist liraglutide in children with obesity.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A single- center prospective study included 20 children aged 12–18 years with obesity grades II–III and morbid obesity (SDS BMI ≥ 2.5). All patients received liraglutide at a standard dose of 3.0 mg for 12 months. Anthropometric parameters (body weight, SDS BMI) and levels of adipokines and incretins (leptin, spexin, ghrelin, obestatin, GLP-1, glucagon, glucose-dependent  insulinotropic polypeptide (GIP), glicentin, oxyntomodulin) were assessed before treatment, after 1, 4, and 12 months of therapy.</p></sec><sec><title>Results</title><p>Results. Liraglutide therapy resulted in a significant reduction in body weight and SDS BMI (p &lt; 0.05). A significant decrease in the concentration of spexin, glucagon-like peptide-1, and obestatin in blood plasma and an increase in the concentration of glucose-dependent  insulinotropic polypeptide and oxyntomodulin in blood serum were detected. Significant correlations were found between SDS BMI dynamics and levels of leptin (r = 0.558; p &lt; 0.001), GLP-1 (r = 0.366; p = 0.026), and obestatin (r = -0.364; p = 0.023).</p></sec><sec><title>Conclusions</title><p>Conclusions. Liraglutide therapy leads not only to weight loss but also to significant changes in the adipokine and incretin profile in children with obesity.
