В обзоре материалов ежегодного собрания 56th ESPGHAN 2024 Annual Meeting, прошедшего в мае 2024 г. в Милане (Италия), были рассмотрены новые направления в диагностике, выявлении факторов риска, таргетном лечении и профилактике целиакии у детей. С докладами выступили ведущие исследователи целиакии из Италии, Германии и США. В частности, профессор R. Tronconi (Италия) в качестве приоритетных направлений в изучении наметил следующие: предотвращение/ослабление прогрессирования целиакии, скрининг для ранней диагностики, неинвазивные маркеры, педиатрические особенности нового лечения, показания к таргетной терапии. Профессор S. Koletzko (Германия) изложила позиции Европейского и Североамериканского общества детских гастроэнтерологов, гепатологов и нутрициологов по таргетному лечению целиакии у детей. Были озвучены основные направления таргетного лечения детей с целиакией. S. Koletzko отметила, что необходима разработка клинических руководств для новых методов лечения целиакии у детей по целевым группам. Желательным вариантом будет первичная профилактика для детей с высоким генетическим риском и вторичная профилактика для потенциальной целиакии. Особое место занимают данные о диагностике целиакии у детей, находящихся на безглютеновой диете. Рассмотрены новые направления в проведении провокации глютеном и возможность проведения биопсии залуковичного отдела двенадцатиперстной кишки без провокации, основанные на иммунофенотипировании лимфоцитов с помощью проточной цитометрии. В обзоре также обобщены данные современной литературы по темам, рассматриваемым на 56th ESPGHAN 2024 Annual Meeting.</p></abstract><trans-abstract xml:lang="en"><p>Celiac disease has gradually become an epidemiologically significant phenomenon, attracting the attention of the scientific community. The review of the materials of the 56th ESPGHAN 2024 Annual Meeting, held in May 2024 in Milan (Italy), considered new directions in diagnostics, identification of risk factors, targeted treatment and prevention of celiac disease in children Presentations were made by leading celiac disease researchers from Italy, Germany and the USA. In particular, Professor R. Tronconi (Italy) outlined the following areas as priority directions in celiac disease research: prevention/reduction of celiac disease progression, screening forearly diagnosis, non-invasive markers, paediatric features of new treatment, indications for target therapy. Prof S. Koletzko (Germany) presented the positions of the European and North American Society of Paediatric Gastroenterologists, Hepatologists and Nutritionists on the target treatment of celiac disease in children. A special place is occupied by data on the diagnosis of celiac disease in children on a gluten-free diet. New directions in conducting gluten provocation and the possibility of performing a biopsy of the retrobulbar part of the duodenum without provocation, based on immunophenotyping of lymphocytes using flow cytometry, are considered.
