Саркоидоз относят к заболеваниям, которые могут приводить к легочному фиброзу, но частота развития прогрессирующего фиброза и необходимость антифибротической терапии до конца не определены.</p></sec><sec><title>Цель</title><p>Цель. Оценка состояния пациентов с саркоидозом на момент формирования у них фиброза легких и в течение двух последующих лет.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Ретроспективный анализ данных 65 пациентов с верифицированным диагнозом «саркоидоз» из медицинских центров Казани, Москвы, Санкт-Петербурга и Иркутска. Фиксировали данные в четырех точках: при выявлении саркоидоза, при обнаружении фиброза и спустя один и два года после. Анализ проведен с помощью программы SPSS-18 (IBM, США). Значимыми принимались различия p &lt; 0,05.</p></sec><sec><title>Результаты</title><p>Результаты. При выявлении стадия I была диагностирована у 7 (10,8%) пациентов, стадия II – у 41 (63,1%), стадия Ш – у 15 (23,1%) и стадия IV – у 2 (3,0%) пациентов. Синдром Лефгрена был у 2 (3,1%) пациентов. Согласно международному Дельфийскому консенсусу, чаще всего встречался фиброз типа А – у 25 (38,5%) человек, типа С и D – по 14 (по 21,5%) и типа B – у 12 (18,5%) человек. Снижение форсированной жизненной емкости легких (ФЖЕЛ) было значимым при выявлении фиброза с 80,0 ± 3,7% до 69,9 ± 3,3% (p = 0,0001), через год – с 72,3 ± до 3,7% (p = 0,015) и через 2 года – с 68,3 ± до 3,5% (p = 0,0001); после образования фиброза снижения не было. Признаки усиления фиброза по данным компьютерной томографии высокого разрешения (КТВР) у 58,5% не изменились, у 30,2% – увеличились, а у 11,3% – уменьшились. Диффузионная способность легких по оксиду углерода (DLco), как и ФЖЕЛ, значимо снизилась относительно исходного значения, но после образования фиброза не снижалась. Только в 50% при выявлении саркоидоза тактика ведения соответствовала федеральным клиническим рекомендациям. Системные глюкокортикостероиды (ГКС) до формирования фиброза получали 66,2%, а затем 43,1% и 34,1% соответственно. Среди препаратов, влиявших на фиброз, применялись нинтеданиб (13,6%), пирфенидон (4,6%), N-ацетилцистеин (6,8%) и лонгидаза (3,1%), ни один из них не повлиял значимо на все показатели прогрессирующего фиброза. На малой выборке не было выявлено преимуществ их применения как при стабильном, так и при прогрессирующем фиброзе у больных саркоидозом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Sarcoidosis is considered a disease that can lead to pulmonary fibrosis, but the incidence of progressive fibrosis and the need for antifibrotic therapy have not been fully determined.</p></sec><sec><title>Аim</title><p>Аim. To evaluate the condition of patients with sarcoidosis at the time of development of pulmonary fibrosis and during the following two years.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. Retrospective analysis of data of 65 patients with verified diagnosis of sarcoidosis from medical centers of Kazan, Moscow, St. Petersburg and Irkutsk. Data were recorded at four points: upon detection of sarcoidosis, upon detection of fibrosis and one and two years later. The analysis was performed using SPSS-18 software (IBM, USA). Differences were considered significant at p &lt; 0.05.</p></sec><sec><title>Results</title><p>Results. Upon detection, stage I was diagnosed in 7 (10.8%), stage II – in 41 (63.1%), stage III – in 15 (23.1%) and stage IV – in 2 (3.0%). Löfgren’s syndrome was present in 2 (3.1%) patients. According to the international Delphi consensus, the most common fibrosis was type A – 25 (38.5%), type C and type D – 14 each (21.5%), and type B – 12 (18.5%). The decrease in FVC was significant upon detection of fibrosis from 80.0 ± 3.7% to 69.9 ± 3.3% (p = 0.0001), after a year 72.3 ± 3.7% (p = 0.015) and after 2 –</p></sec><sec><title>68</title><p>68.3 ± 3.5% (p = 0.0001), after the formation of fibrosis there was no decrease. HRCT signs did not change in 58.5%, increased in 30.2%, and decreased in 11.3%. DLco, like FVC, significantly decreased relative to the initial value, but did not decrease after the formation of fibrosis. Only in 50% of cases of sarcoidosis, the treatment tactics corresponded to federal clinical recommendations. Systemic GCS were received by 66.2% before the formation of fibrosis, and then by 43.1% and 34.1%. Among the drugs that affected fibrosis, nintedanib (13.6%), pirfenidone (4.6%), N-acetylcysteine (6.8%) and longidaza (3.1%) were used, none of them significantly affected all the indicators of progressive fibrosis.
