Тошнота и рвота, индуцированные химиотерапией, остаются одним из наиболее неприятных и опасных побочных эффектов. Нетупитант/палоносетрон (Акинзео) представляют собой эффективную и удобную в применении комбинацию лекарственных средств для профилактики тошноты и рвоты у пациентов, получающих химиотерапию с умеренным или высоким эметогенным потенциалом.</p></sec><sec><title>Цель</title><p>Цель. Изучить эффективность применяемых в клинической практике схем профилактики тошноты и рвоты, ассоциированных с высокоэметогенной противоопухолевой химиотерапией рака мочевого пузыря (нетупитант/палоносетрон и апрепитант/ондансетрон).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проспективно проанализирован опыт сопроводительной антиэметической терапии 47 пациентов, получавших противоопухолевую химиотерапию рака мочевого пузыря по схеме «гемцитабин + цисплатин». Показатели ответа были рассчитаны в острой (0–24 ч), отсроченной (24–120 ч) и общей фазах первого цикла введения.</p></sec><sec><title>Результаты и обсуждение</title><p>Результаты и обсуждение. Частота полного ответа при пероральном применении нетупитанта/палоносетрона и апрепитанта/ондансетрона достигла 86,9% и 87,5% – в острой (р = 0,2), 82,6% и 75,0% – в отсроченной (р = 0,02) и 78,3% и 70,8% – в общей (р = 0,04) фазе соответственно. Прорывная рвота в течение 120 ч после введения цисплатина имела место у 17,4% пациентов группы нетупитанта/палоносетрона и у 20,8% пациентов группы апрепитанта/ондансетрона (р = 0,25). Все случаи прорывной рвоты развивались с 3-х по 5-е сут. Частота развития нежелательных побочных явлений была сопоставимой между группами нетупитанта/палоносетрона (65,1%) и апрепитанта/ондансетрона (62,3%). Большинство (95,7%) пациентов оценивали нежелательные побочные явления как легкой/умеренной интенсивности. За время наблюдения летальных исходов не было. Несмотря на малый объем выборки, полученные нами результаты соответствуют современным данным литературы.</p></sec><sec><title>Выводы</title><p>Выводы. Нетупитант/палоносетрон обладают высокой эффективностью в профилактике индуцированной химиотерапией тошноты и рвоты. Пероральная форма и однократный прием выгодно отличают препарат от более сложных схем антиэметической терапии предыдущего поколения. Современная антиэметическая профилактика позволяет улучшить качество жизни большинства пациентов, однако необходимо дальнейшее изучение проблемы индуцированной химиотерапией тошноты и рвоты.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Chemotherapy-induced nausea and vomiting remain among the most unpleasant and dangerous side effects. Netupitant/palonosetron (Akynzeo) is an effective and convenient combination therapy for the prevention of nausea and vomiting in patients receiving chemotherapy with moderate to high emetogenic potential.</p></sec><sec><title>Aim</title><p>Aim. To evaluate the efficacy of clinically used regimens for the prevention of nausea and vomiting associated with highly emetogenic antitumor chemotherapy for bladder cancer (netupitant/palonosetron and aprepitant/ondansetron).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We prospectively analyzed the experience of concomitant antiemetic therapy in 47 patients receiving gemcitabine-cisplatin antitumor chemotherapy for urothelial carcinoma of the bladder. Response rates were calculated in the acute (0–24 h), delayed (24–120 h), and overall phases of the first GC cycle.</p></sec><sec><title>Results and discussion</title><p>Results and discussion. Complete response rates with oral netupitant/palonosetron and aprepitant/ondansetron were 86.9% and 87.5% in the acute (p = 0.2), 82.6% and 75.0% in the delayed (p = 0.02), and 78.3% and 70.8% in the overall (p = 0.04) phases, respectively. Breakthrough vomiting within 120 hours after cisplatin administration occurred in 17.4% of patients in the netupitant/palonosetron group and 20.8% in the aprepitant/ondansetron group (p = 0.25). All cases of breakthrough vomiting developed between days 3 and 5. The incidence of adverse events (AEs) was comparable between the netupitant/palonosetron (65.1%) and aprepitant/ondansetron (62.3%) groups. Most patients (95.7%) rated their AEs as mild to moderate. There were no deaths during the follow-up period. Despite the small sample size, our results are consistent with the current literature data.</p></sec><sec><title>Conclusions</title><p>Conclusions. Netupitant/palonosetron is highly effective in the prevention of chemotherapy-induced nausea and vomiting. Its oral administration and single dose make it unique, distinguishing it from more complex previous-generation antiemetic therapy regimens.
