Развитие фебрильной нейтропении (ФН) ассоциировано с высокой летальностью и значительными затратами для системы здравоохранения. В настоящее время при режимах химиотерапии (ХТ) с промежуточным риском развития осложнений (10–20%) применение гранулоцитарных колониестимулирующих факторов (Г-КСФ) рекомендуется пациентам, имеющим как минимум один клинический фактор риска развития ФН. Остается открытым вопрос о том, насколько оптимально проводится профилактика ФН и как использование Г-КСФ влияет на бюджет лечебно-профилактического учреждения (ЛПУ) при терапии пациентов с промежуточным риском развития осложнений.</p></sec><sec><title>Цель</title><p>Цель. Оценить влияние первичной профилактики ФН на затраты ЛПУ, а также на показатели выживаемости у пациентов с промежуточным риском осложнений при проведении ХТ.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен ретроспективный анализ 252 историй болезни пациентов (средний возраст – 66,8 года; 52% женщин, 48% мужчин). Разработана модель влияния на бюджет ЛПУ, учитывающая прямые медицинские затраты на профилактику, лабораторно-инструментальные исследования и лечение эпизодов нейтропенических осложнений, а также наступление смерти пациента к моменту выполнения анализа.</p></sec><sec><title>Результаты</title><p>Результаты. Прямые затраты на одного пациента при проведении профилактики ФН составили 53 095,26 руб., что оказалось на 6% (3 212,16 руб.) выше, чем в группе без проведения профилактики. Первичная профилактика ФН привела к снижению числа эпизодов нейтропении. Разница рисков в группе с профилактикой ФН составила -26,1 (95% ДИ -35,6; -14,1), p &lt; 0,001. В результате одногодичная выживаемость в группе с профилактикой нейтропении оказалась статистически значимо выше – 66,1% (41 из 62), при этом затраты на одного пациента были на 33% (19 840,44 руб.) ниже, чем в группе без проведения профилактики.</p></sec><sec><title>Заключение</title><p>Заключение. Назначение первичной профилактики с использованием Г-КСФ связано со снижением частоты развития нейтропении и улучшением исходов непосредственной и отдаленной эффективности, что положительно влияет на бюджет учреждения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. The development of febrile neutropenia (FN) is associated with high mortality and significant healthcare costs. Currently, granulocyte colony-stimulating factors (G-CSF) are recommended for patients with at least one clinical risk factor for FN in chemotherapy (CT) regimens with an intermediate risk of complications (10–20%). The question remains open as to how optimally FN prevention is carried out and how the use of G-CSF affects the budget of a medical institution in the treatment of patients with an intermediate risk of complications.</p></sec><sec><title>Aim</title><p>Aim. To assess the impact of primary FN prevention on the costs of a medical institution and on survival rates in patients undergoing intermediate-risk CT regimens.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospective analysis of 252 patient medical records was conducted (average age 66.8 years; 52% women, 48% men). A model was developed to assess the impact on the budget of a medical institution, taking into account direct medical costs for prevention, laboratory and instrumental studies, and treatment of episodes of neutropenic complications, as well as patient death by the time of analysis.</p></sec><sec><title>Results</title><p>Results. Direct costs per patient for FN prevention amounted to 53,095.26 RUB and were 6% higher (3,212.16 RUB) compared to the group without prevention. Primary FN prevention led to a reduction in episodes of neutropenia, with a risk difference in the FN prevention group -26.1 (95% CI -35.6; -14.1), p &lt; 0.001. As a result, one-year survival in the neutropenia prevention group was statistically significantly lower at 66.1% (41 out of 62), and costs per patient were 33% lower (19,840.44 RUB) compared to the group without prevention.</p></sec><sec><title>Conclusion</title><p>Conclusion.
