Комбинация атезолизумаба, бевацизумаба и химиотерапии (АБПК) – стандарт лечения распространенного неплоскоклеточного немелкоклеточного рака легкого (НМРЛ), в том числе после прогрессирования на ингибиторах тирозинкиназ (ИТК) при EGFR/ALK мутациях. Однако данные об эффективности АБПК в реальной практике в РФ ограничены.</p></sec><sec><title>Цель</title><p>Цель. Оценить общую выживаемость (ОВ) и выживаемость без прогрессирования (ВБП) у пациентов с НМРЛ в реальной практике, получивших ≥4 циклов АБПК в 1-й линии и перешедших на поддерживающую терапию атезолизумабом и бевацизумабом (АБ).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. REACT – наблюдательное ретроспективное многоцентровое исследование. Включались взрослые пациенты с распространенным НМРЛ (которым диагноз был установлен в 2021–2023 гг.), получившие ≥4 циклов АБПК и начавшие поддержку АБ. Ключевые конечные точки: частота объективного ответа (ЧОО), ВБП и ОВ.</p></sec><sec><title>Результаты</title><p>Результаты. Включены данные 108 пациентов: 78 пациентов (72,2%), получавших АБПК в 1-й линии (без мутаций), и 30 (27,8%) после ИТК (EGFR/ALK+). Медиана возраста 57,6 года, 63,9% мужчин. Исходные метастазы: ЦНС – 17,6%, печень – 8,3%, кости – 33,3%. ЧОО во всей выборке 56,5% (59,0% в 1-й линии), частота контроля заболевания – 98,2%. Медиана ВБП в 1-й линии – 14,7 мес. (95% ДИ 12,4–23,9), после ИТК – 9,3 мес. (95% ДИ 7,4–14,6). Медиана ОВ в 1-й линии – 34,4 мес. (95% ДИ 21,8–НД), после ИТК – 24,2 мес. (95% ДИ 21,8–НД). Двухлетняя ОВ – 52,7 и 54,4% соответственно. Высокая опухолевая нагрузка (≥3 очагов), большая сумма диаметров очагов и метастазы в костях были негативными прогностическими факторами. Возраст, пол, курение, экспрессия PD-L1 не показали значимого влияния.</p></sec><sec><title>Заключение</title><p>Заключение. В данной когорте пациентов режим АБПК показал высокую эффективность. Результаты согласуются с IMpower150 для группы после ИТК и демонстрируют более длительную выживаемость в 1-й линии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Atezolizumab, bevacizumab, and chemotherapy (ABCP) is a standard of care for advanced non-squamous NSCLC, including post-TKI patients with EGFR/ALK mutations. Real-world data on ABCP efficacy in Russia are limited.</p></sec><sec><title>Aim</title><p>Aim. To evaluate OS and PFS in real-world NSCLC patients who received ≥4 cycles of first-line ABCP followed by atezolizumab-bevacizumab (AB) maintenance.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. REACT is an observational, retrospective, multicenter study of adult patients with advanced NSCLC (diagnosed 2021–2023) who received ≥4 ABCP cycles and started AB maintenance. Key endpoints included ORR, PFS, and OS.</p></sec><sec><title>Results</title><p>Results. Data from 108 patients were included: 78 patients (72.2%) received ABCP in first line (wild type) and 30 (27.8%) were post-TKI (EGFR/ALK+). Median age 57.6 years; 63.9% male. Baseline metastases: CNS (17.6%), liver (8.3%), bone (33.3%). ORR was 56.5% (59.0% first line); DCR 98.2%. Median PFS was 14.7 months (95% CI 12.4–23.9) in first line group and 9.3 months (95% CI 7.4–14.6) in post-TKI group. Median OS was 34.4 months (95% CI 21.8–NR) in first line group and 24.2 months (95% CI 21.8–NR) post-TKI. 2-year OS rates were 52.7% and 54.4%, respectively. High tumor burden (≥3 sites), large sum of longest diameters, and bone metastases were negative prognostic factors. Age, sex, smoking, and PD-L1 status had no significant impact.</p></sec><sec><title>Conclusion</title><p>Conclusion. In this cohort, the ABCP regimen was highly effective.
