Инвалидность, связанная с инсультом, может быть классифицирована на двигательные и немоторные нарушения. Двигательные нарушения, такие как гемиплегия, являются наиболее заметными последствиями. Тем не менее немоторные нарушения, такие как когнитивный дефицит, нарушение речи, зрительные расстройства, дисфагия, расстройства настроения и боль, могут привести к гораздо большему снижению качества жизни. В статье описан случай пациента С. 59 лет, поступившего в отделении медицинской реабилитации (ОМР) пациентов с нарушением функции центральной нервной системы, через 1 мес. после развития повторного ишемического инсульта в бассейне основной артерии, с развитием грубой дизартрии, грубой дисфагии, правосторонней гемиплегии, гемигипестезии. В связи с наличием у пациента грубой дисфагии и невозможностью приема пищи через рот пациент поступал в ОМР с уже установленным назогастральным зондом. На 5-й день от поступления в ОМР пациенту проведена ботулинотерапия препаратом ботулинический токсин типа А под УЗИ-навигацией в околоушные железы и подчелюстные слюнные железы билатерально. В результате проведенного комплексного лечения (логопедическая и физическая реабилитация, ботулинотерапия, медикаментозная терапия) у пациента улучшилась функция глотания: увеличился объем проглатываемой пищи, уменьшилось количество поперхиваний при глотании, усилился кашлевой и восстановился глоточный рефлекс, появилась возможность самостоятельно откашливать слизь, улучшилась фонация мягкого неба, уменьшилась выраженность дизартрии, двигательный режим был расширен.</p></abstract><trans-abstract xml:lang="en"><p>Stroke is a major cause of disability for adults. Stroke-related disability can be categorized into motor and non-motor impairments. Motor impairments, such as hemiplegia, are the most noticeable sequelae. However, non-motor impairments, such as cognitive deficits, speech impairment, visual disorders, dysphagia, mood disorders, and pain can cause far greater decreases in quality of life. This article presents a case of patient S., 59 years old, admitted to the Rehabilitation Department for patients with central nervous system dysfunction. The patient was hospitalized one month after recurrent basilar artery ischemic stroke, which resulted in the severe dysarthria, severe dysphagia, right hemiplegia, and hemihypesthesia. Due to severe dysphagia and failure of food passage from the mouth to the stomach, the patient had an existing NG tube in place when he was admitted to the Rehabilitation Department. On the fifth day of admission to the Rehabilitation Department, the patient underwent ultrasonography-guided Botulinum neurotoxin type A (BoNT-A) injections into the submandibular and parotid glands bilaterally. As a result of the comprehensive treatment (speech and physical rehabilitation, botulinum therapy, drug therapy), the patient showed improvement in swallowing function: an increase in the swallowed bolus volume, a decrease in the number of choking sensations when swallowing, enhancement of the cough reflex and restoration of the pharyngeal reflex, ability to spontaneously cough up mucus, improvement of soft palate phonation, reduction in dysarthria severity, and expansion of his motor activities.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>ишемический инсульт</kwd><kwd>дисфагия</kwd><kwd>сиалорея</kwd><kwd>аспирация</kwd><kwd>реабилитация</kwd></kwd-group><kwd-group xml:lang="en"><kwd>ischemic stroke</kwd><kwd>dysphagia</kwd><kwd>sialorrhea</kwd><kwd>aspiration</kwd><kwd>rehabilitation</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Feigin VL, Brainin M, Norrving B, Martins SO, Pandian J, Lindsay P et al.
