Эффективность и безопасность сатрализумаба в лечении заболеваний спектра оптиконевромиелита (ЗСОНМ) продемонстрированы в рандомизированных клинических исследованиях и продолжают анализироваться в реальной клинической практике (РКП).</p></sec><sec><title>Цель</title><p>Цель. Проанализировать опыт применения сатрализумаба у российских пациентов с ЗСОНМ в РКП.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследование было многоцентровым ретроспективным с анализом данных из амбулаторных карт/ историй болезни. Оценивались исходные клинико-демографические характеристики пациентов и результаты по эффективности и безопасности их лечения сатрализумабом.</p></sec><sec><title>Результаты</title><p>Результаты. Проанализированы данные 71 пациента в возрасте от 12 до 69 лет в начале лечения сатрализумабом. Все пациенты были с ЗСОНМ с антителами к аквапорину-4, 18,3% пациентов имели высокую активность по частоте обострений, 69% пациентов получали до сатрализумаба препараты, предупреждающие обострения, у 41% пациентов использовались «малые дозы» глюкокортикоидов на старте лечения им. Среднегодовая частота обострений (СЧО) за последние 2 года до лечения была 1,26. Длительность терапии сатрализумабом составила 15 [12; 24,5] мес. На фоне терапии у 58 (82%) пациентов не было обострений, из них у 9 (16%) с высокоактивным ЗСОНМ. СЧО снизилось до 0,11 за 2 года терапии (p &lt; 0,001). У 8 (62%) из 13 пациентов с обострениями оно развилось до 6 мес. лечения, у 6 (46,2%) – в «кластерный период», 11 (84,6%) пациентов не получали «малые дозы» глюкокортикоидов или их отменили до 6 мес. терапии. У 19 (26,8%) пациентов зарегистрированы нежелательные явления, нейтропения встречалась наиболее часто. У 27 (84,4%) из 32 пациентов после переключения с ритуксимаба не было обострений при благоприятном профиле безопасности.</p></sec><sec><title>Выводы</title><p>Выводы. Опыт применения сатрализумаба в РКП продемонстрировал эффективность и благоприятный профиль безопасности у российских пациентов с ЗСОНМ с антителами к аквапорину-4 в возрасте 12 лет и старше.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Effectiveness and safety of satralizumab in the treatment of neuromyelitis optica spectrum disorders (NMOSD) have been demonstrated in randomized clinical trials and continue to be evaluated in the real clinical practice (RCP).</p></sec><sec><title>Aim</title><p>Aim. To analyze the experience of satralizumab administration in Russian patients with NMOSD in the RCP.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study was a multicenter retrospective. Analyzed data were taken from outpatient charts/medical records. Baseline clinical and demographic characteristics of patients with results on efficacy and safety of satralizumab treatment were assessed.</p></sec><sec><title>Results</title><p>Results. Data of 71 patients with ages ranging from 12 to 69 years at the start of satralizumab treatment were analyzed. 100% of patients had NMOSD with antibodies to aquaporin-4. At baseline 18.3% of patients were with highly active NMOSD by exacerbation frequency, 69% had previous relapse preventive treatment, 41% received low-dose glucocorticoids. The annualized relapse rate (ARR) during previous 2 years before treatment was 1.26. The median duration of satralizumab therapy was 15 [12; 24.5] months. 58 (82%) patients were relapse-free and included 9 (16%) persons with highly active NMOSD. The ARR decreased to 0.11 over 2 years of therapy (p &lt; 0.001). In 8 (62%) of 13 patients with relapses first attack developed before 6 months of treatment and in 6 (46.2%) cases – during the “clustered period”, 11 (84.6%) patients did not receive low doses glucocorticoids or they were discontinued before 6 months of satralizumab therapy. Adverse events were reported in 19 (26.8%) patients. The neutropenia was the most common. 27 (84.4%) of 32 patients switched from rituximab to satralizumab were relapse-free with a favorable safety.</p></sec><sec><title>Conclusions</title><p>Conclusions.
