Фармакотерапия аллергического ринита

Аннотация

Клиническая картина аллергического ринита включает чихание, выделение из носа, нарушение носового дыхания, а также ощущение зуда в носу и на небе. В зависимости от длительности симптомов аллергический ринит подразделяется на интермиттирующую и персистирующую формы, а по выраженности симптомов – на легкую, среднетяжелую и тяжелую формы. Лечение аллергического ринита включает устранение контакта с аллергеном, аллерген-специфическую иммунотерапию и фармакотерапию. Подбор схемы лечения определяется тяжестью симптомов, возрастом пациента и наличием сопутствующих заболеваний. Целью работы является обзор наиболее часто применяемых фармакопрепаратов при аллергическом рините. Солевые растворы используются как монотерапия, когда симптомы минимальны, или же перед применением других топических препаратов для очищения слизистой перед их нанесением. Интраназальные глюкокортикостероиды могут использоваться в качестве монотерапии при разной степени тяжести симптомов и дополняться другими препаратами при недостаточной эффективности. Интраназальные глюкокортикостероиды не оказывают системного воздействия из-за минимальной биодоступности. При среднетяжелой и тяжелой формах аллергического ринита рекомендовано их совместное использование с антигистаминными препаратами 2-го поколения, интраназальными кромонами, интраназальными антигистаминными препаратами и антагонистами лейкотриеновых рецепторов. Значимыми побочными эффектами антигистаминных препаратов, особенно 1-го поколения, является седативное воздействие на ЦНС, а антагонистов лейкотриеновых рецепторов – нервнопсихические нарушения. Интраназальные кромоны имеют высокий профиль безопасности, что определяет их популярность в детской практие, однако схема дозирования до 3–4 раз в день уменьшает приверженность пациентов к лечению. Деконгестанты не показаны в качестве монотерапии при аллергическом рините, но могут использоваться в сочетании с другими препаратами короткими курсами с учетом возможного развития побочных эффектов.
