Применение колхицина при подагре

Аннотация

Колхицин - препарат, известный на протяжении нескольких тысячелетий, еще со времен Древнего Египта, но успешно используется для лечения подагры до настоящего времени. В основе действия колхицина лежит повреждение микротрубочек, подавление митоза, а также способность ингибировать активацию кристаллами моноурата натрия NLRP3 инфламмасомы и блокировать тем самым высвобождение интерлейкина (ИЛ)-1р, ключевого цитокина в развитии подагрического воспаления. Тем не менее механизм действия колхицина до сих пор не до конца изучен.

Колхицин следует рассматривать не только как оптимальное лекарственное средство для купирования острого приступа артрита при подагре, но и как наилучший метод профилактики приступов подагрического артрита при подборе уратснижаю-щей терапии. Последние исследования подтвердили хороший профиль эффективности и безопасности препарата при его правильном применении у пациентов с подагрой. В настоящее время рекомендовано применять низкие дозы препарата (1-1,5 мг/сут для купирования острого приступа артрита и 0,5-1,0 мг с целью профилактики приступов при инициации урат-снижающей терапии). При этом, по результатам последних исследований, по эффективности низкие дозы не уступают высоким, но при использовании низких доз частота развития нежелательных эффектов намного меньше. В отличие от нестероидных противовоспалительных препаратов и глюкокортикоидов препарат не оказывает негативного влияния на сердечнососудистую систему и может применяться у пациентов с сахарным диабетом. Это позволяет использовать препарат длительно, что особенно важно, так как рекомендуется проводить профилактику приступов артрита на протяжении как минимум 6 мес. от начала приема уратснижающих препаратов. Это позволяет избежать обострения артрита у большинства пациентов и многократно снизить их частоту. Кроме того, в свете накапливающихся данных о влиянии препарата на сердечно-сосудистую систему, а также благодаря высокому профилю безопасности следует рассматривать и другие точки приложения, где могут быть использованы как противовоспалительные, так и кардиопротективные свойства колхицина.
