Оптимизация ведения пациентов с персистирующим постуральным перцептивным головокружением и мигренью

Введение. Головокружение и головная боль относятся к самым частым жалобам в неврологической практике, которые значительно снижают качество жизни пациентов, поэтому разработка эффективных методов ведения пациентов с персистирующим постуральным перцептивным головокружением (ПППГ) и мигренью является актуальной задачей.

Цель. Изучить и оптимизировать типичную практику ведения пациентов с ПППГ и мигренью.

Материалы и методы. Обследовано 22 пациента в возрасте 39,3 ± 10,2 года с ПППГ согласно диагностическим критериям ПППГ общества Барани и мигренью. В ходе исследования использовались: госпитальная шкала тревоги и депрессии (Hospital Anxiety and Depression scale), шкала депрессии Бека (Beck Depression Inventory), шкала тревоги Спилбергера (State-Trait Anxiety Inventory), клиническое отоневрологическое обследование, отоневрологический опросник, шкала оценки головокружения (Dizziness Handicap Inventory), видеонистагмография. После постановки диагноза пациентам было назначено комплексное лечение. Для купирования острого приступа головокружения назначались дименгидринат, а также комбинированный препарат циннаризина 20 мг + дименгидрината 40 мг Арлеверт®, который в ряде исследований показал высокую эффективность и хорошую переносимость. Через месяц было проведено обследование пациентов в динамике.

Результаты. У пациентов с ПППГ были выявлены мигрень без ауры (54%), мигрень с аурой (14%) и вестибулярная мигрень (32%). Уровень тревоги был достоверно выше в группе пациентов с ПППГ и вестибулярной мигренью. Отмечалась умеренная выраженность головокружения во всех группах пациентов, через месяц на фоне проводимой терапии выраженность головокружения достоверно уменьшилась во всех группах. Диагнозы «ПППГ» и «вестибулярная мигрень» не были ранее установлены ни одному из обследованных нами пациентов.

Выводы. Исследование показало низкий уровень диагностики ПППГ и вестибулярной мигрени. Ведение пациентов с ПППГ и мигренью требует комплексного подхода.
