Основные аспекты приверженности лечению пероральными противоопухолевыми препаратами: обзор литературы

Не менее 25% противоопухолевых препаратов предназначены для перорального применения, при этом создание и быстрое внедрение пероральных препаратов в клиническую практику постоянно растет. За период с 2006 по 2021 г. одобрено 80 пероральных форм противоопухолевых препаратов. Учитывая, что при равной эффективности и токсичности пациенты предпочитают пероральную терапию внутривенной при возможности такой замены, вопрос приверженности лечению пероральными формами открыт и актуален для изучения. На сегодняшний день приверженность, как основной фактор в достижении лучшего клинического результата, изучена с разных позиций, однако глобально нет структурированных данных, а соответственно, доказательных и конкретизированных рекомендаций при отсутствии приверженности. В обзор включены данные исследований, выполненных за последние 10 лет, изучающих приверженность пациентов при лечении пероральными формами противоопухолевых препаратов. Источниками для обзора послужили базы данных Medline, Embase и Scopus. Анализ литературы показал, что наиболее важными аспектами в области приверженности лечению пероральными противоопухолевыми препаратами являются: уровень приверженности пациентов лечению, факторы, влияющие на несоблюдение приверженности, корреляция приверженности лечению и клинических исходов при злокачественных новообразованиях. Вместе с тем в статье рассмотрено влияние несоблюдения приверженности на систему здравоохранения в целом. Изучены наиболее достоверные и современные данные в исследуемой области для последующего определения мер, направленных на достижение наилучшей практики в организации здравоохранения пациентов со злокачественными новообразованиями. Результаты исследований демонстрируют вариабельность и разнородность данных, отсутствие единых методологических подходов, что препятствует формированию объективных и достоверных выводов о приверженности. Инициация масштабных исследований, принятие единых стандартных процедур и рекомендаций по оценке приверженности позволят принимать рациональные стратегические решения по контролю приверженности лечению.
