Сложности дифференциальной диагностики фибромиалгии

Хроническая распространенная боль является ведущим признаком фибромиалгии (ФМ). Однако она характерна не только для ФМ; другие заболевания могут также проявляться хроническим болевым синдромом. Это требует проведения дифференциальной диагностики. Последняя представляет собой непростую задачу, поскольку, с одной стороны, необходимо знать клиническую картину ФМ, современные диагностические критерии и уметь пользоваться ими на практике. Согласно опросу среди врачей на знание критериев диагноза ФМ, только 10% специалистов смогли воспроизвести критерии диагностики. С другой стороны, врач должен помнить перечень тех состояний, которые маскируют ФМ. Заболевания, которые могут сопутствовать или имитировать ФM, включают в себя ревматологическую, неврологическую, скелетно-мышечную, эндокринную, гастроинтестинальную, инфекционную патологию, а также могут быть связаны с приемом лекарств. Сопутствующая психиатрическая патология ухудшает течение ФМ. Затрудняет диагностический поиск отсутствие на сегодняшний день четко определенного биомаркера ФМ. В статье представлены современные критерии ФМ Американской коллегии ревматологов (American College of Rheumatology, AСR) 2011 и 2016 гг., показавшие большую диагностическую валидность по сравнению с критериями ААРТ 2019 г. Согласно современной концепции, ФМ является диагнозом исключения, но это не исключает наличия нескольких нозологий одновременно. Также раскрыты основные диагностические моменты дифференциального поиска. Знание принципов дифференциальной диагностики способствует своевременной постановке правильного диагноза и повышает шанс успешного лечения пациентов с хроническим болевым синдромом. Цель работы – провести обзор литературы о критериях диагностики и заболеваниях, которые могут имитировать ФМ, и выделить признаки, которые могут отличать эти заболевания от ФМ.
