Станет ли неоадъювантная иммунотерапия новой парадигмой лечения больных раком легкого

Неоадъювантная иммунотерапия – одно из основных исследовательских направлений в онкологии, которое позволяет продвинуться в понимании механизмов действия и поиске предиктивных факторов эффективности иммунотерапии. В раке легкого этот интерес подкреплен успехами монотерапии иммунологическими препаратами и комбинированной иммунохимиотерапии при метастатических формах. Постулат, что неоадъювантной иммунотерапией достигается девитализация опухоли перед хирургическим вмешательством, подтверждается морфологической оценкой операционного материала с градацией противоопухолевого эффекта. Кроме влияния непосредственно на опухолевый очаг, оказывается воздействие на потенциальные микрометастазы. Морфологическая оценка с градацией противоопухолевого эффекта (MPR – значимый патоморфологический ответ, CPR – полный патоморфологический эффект) дает уникальную возможность выявить предикторы эффективности и скорректировать лечебную тактику. Значимый морфологический ответ (MPR – менее 10% жизнеспособных опухолевых клеток в резецированном образце) ассоциирован с лучшей общей выживаемостью. В настоящий момент имеются данные, что назначение неоадъювантной иммунотерапии в монорежиме позволяет достичь MPR в 18–45%, а применение комбинации иммунопрепаратов с химиотерапией увеличивает данный показатель до 32–92%. Неоадъювантная иммунотерапия или ее комбинация с химиотерапией не откладывает хирургическое лечение, но позволяет надеяться на восстановление собственного иммунологического надзора организма и длительного стойкого противоопухолевого эффекта или даже излечения.
