Современные возможности лечения пациенток с ректовагинальными свищами: обзор литературы

Обзор посвящен наиболее актуальным вопросам этиологии, классификации, диагностики и лечения ректовагинальных свищей. Невозможно переоценить психосоциального воздействия ректовагинального свища на  качество жизни женщин. Наиболее частыми причинами возникновения ректовагинальных свищей являются послеоперационные и  акушерские осложнения, воспалительные заболевания кишечника, осложнения лучевой терапии и новообразования. Сочетание передовых методов визуализации, физикального обследования и клинического опыта может помочь в определении местоположения и причины возникновения свища. Терапевтические возможности разнообразны, однако их результаты часто неудовлетворительны. В связи с этим выбор стратегии лечения ректовагинальных свищей должен основываться на этиологии, локализации и сопутствующей патологии. Несмотря на распространенность и серьезные последствия ректовагинальных свищей, не существует общепринятого научно обоснованного подхода к хирургическому лечению. Благодаря сотрудничеству между акушерами-гинекологами и колоректальными хирургами, детальной предоперационной оценке, рассмотрению оптимальных вариантов лечения и правильной хирургической технике у большинства пациентов возможно достичь успешного результата. Цель обзора – оценить на основе литературных источников проблему диагностики и лечения пациенток с ректовагинальными свищами и современные возможности улучшения результатов оперативных вмешательств при данной патологии. Методической основой проведенного анализа послужило изучение отечественной и зарубежной литературы, посвященной данной проблеме. Были отобраны научные статьи за период 2016–2021 гг. c наиболее крупными рандомизированными контролируемыми исследованиями в этой области. В статье представлен обзор основных стратегий ведения и  технических подходов, которые в  настоящее время являются эффективными в  устранении данного социально-инвалидизирующего состояния.
