Профилактика нежелательных явлений при грудном вскармливании больных и недоношенных новорожденных

Аннотация
Введение. Грудное вскармливание является критически важным фактором для выживания, здоровья и развития больных и недоношенных новорожденных. Однако данная группа детей сталкивается с наибольшим риском его преждевременного прекращения. Нежелательные явления, такие как нарушение становления лактации у матери, трудности с координацией сосания-глотания-дыхания у ребенка и отсутствие стандартизированных протоколов перехода с зондового питания, являются основными препятствиями.
Цель. Проанализировать современные научные данные о стратегиях профилактики нежелательных явлений нарушения грудного вскармливания больных и недоношенных новорожденных.
Материалы и методы. Поиск литературных источников проводился в электронных базах данных PubMed/MEDLINE, eLIBRARY.RU, Cochrane Library, Google Scholar за период с 1 января 2010 г. по 31 декабря 2025 г. Оценены 156 публикаций, из них включено в обзор 80 источников литературы. Поиск осуществлялся по следующим ключевым словам и их комбинациям на русском и английском языках: «грудное вскармливание/breastfeeding», «недоношенные новорожденные/premature newborns», «больные новорожденные/sick newborns» и др. Дополнительно проводился ручной поиск по спискам литературы релевантных обзоров.
Результаты. На основе анализа отобранных источников литературы делается вывод о необходимости комплексного подхода, включающего раннее начало сцеживания, использование методов кенгуру, объективный мониторинг лактации, в т. ч. по уровню натрия, поддержку непитательного сосания и внедрение четких, поэтапных клинических алгоритмов перехода к грудному вскармливанию. Интеграция этих методов в ежедневную практику отделений интенсивной терапии новорожденных позволяет существенно повысить частоту и продолжительность успешного грудного вскармливания в этой уязвимой популяции.
Заключение. Профилактика нежелательных явлений при грудном вскармливании больных и недоношенных новорожденных нуждается в многоуровневом, научно обоснованном подходе, интегрирующем последние достижения науки и практики.
