Роль прикорма в программировании пищевого поведения

Аннотация

Расстройства пищевого поведения у детей представляют собой актуальную междисциплинарную проблему, требующую комплексного научного анализа. Избегающее/ограничительное расстройство пищевого поведения (ИОРПП) является самостоятельной нозологической единицей в классификации РПП. Для данного обзора поиск литературных источников проводился в электронных базах данных PubMed/MEDLINE, eLIBRARY.RU, Cochrane Library, Google Scholar за период с 2001 по 2026 г. В итоге было отобрано 76 источников литературы. При анализе выявлено, что в исследованиях часто изучаются нейробиологические модели ИОРПП, которые ассоциированы с клиническими особенностями пациентов. Понимание клинических подтипов позволяет оценить проблему ИОРПП с точки зрения этиопатогенетических механизмов и выявить факторы риска заболевания. Критически важным является проведение дифференциальной диагностики ИОРПП и других расстройств пищевого поведения, включая пищевую избирательность, пищевую неофобию и детское расстройство питания. Разграничение данных терминов выступает фундаментальным этапом для определения патогенетической сущности нарушений. Известно, что формирование модели пищевого поведения детерминировано совокупностью факторов риска различной этиологии. Минимизация воздействия факторов риска через призму становления модели пищевого поведения становится возможной лишь при глубоком понимании их биопсихосоциальной природы. Соблюдение определенных правил введения продуктов прикорма является эффективной профилактической мерой в отношении пищевой избирательности и расстройств пищевого поведения у детей. Родители зачастую неосознанно могут фиксировать патологические паттерны пищевой избирательности. Ошибки, включая недостаточную толерантность к отказам, игнорирование возрастных особенностей текстур, психоэмоциональное давление и неоптимальную тактику представления новых продуктов, формируют замкнутый круг избегания и селективного потребления. В статье рассматриваются профилактические меры в отношении пищевой избирательности, включающие методы ассоциативного обучения, и подчеркивается важность выбора оптимальных блюд прикорма.
