Антитромбоцитарная терапия во вторичной профилактике ишемического инсульта

Аннотация
Представлен обзор литературы по антитромбоцитарной (антиагрегантной) терапии во вторичной профилактике ишемического инсульта (ИИ). Анализируются данные последних (2014) рекомендаций Американской ассоциации кардиологов и специалистов по инсульту. Отмечается эффективность ацетилсалициловой кислоты (АСК), дипиридамола замедленного высвобождения в комбинации с АСК, клопидогрела, небольшое преимущество комбинации дипиридамола замедленного высвобождения и АСК, а также клопидогрела над АСК в отношении снижения риска повторных ишемических событий. Обсуждается возможность применения комбинации клопидогрела и АСК у больных, перенесших тран-зиторную ишемическую атаку или малый ИИ, в первые 90 дней с момента заболевания. Анализируются проблемы использования антитромбоцитарной терапии в реальной клинической практике. Отмечается, что использование в клинической практике международных рекомендаций по вторичной профилактике ИИ позволит снизить заболеваемость инсультом и смертность от него.
