ПЕРВИЧНАЯ ПРОФИЛАКТИКА СЕРДЕЧНО-СОСУДИСТЫХ ЗАБОЛЕВАНИЙ И АНТИТРОМБОЦИТАРНАЯ ТЕРАПИЯ. КЛИНИЧЕСКАЯ ЛЕКЦИЯ

Аннотация

Лечение низкими дозами ацетилсалициловой кислоты (АСК) (в монотерапии или в комбинации с другими антитромбоцитарными препаратами) на сегодняшний день является основой вторичной профилактики при ишемической болезни сердца. Длительная терапия низкими дозами АСК рекомендована больным со стабильной ишемической болезнью сердца, у пациентов, перенесших острый коронарный синдром с подъемом и без подъема сегмента ST, после реваскуляризации [1–3]. Необходимость антитромботической терапии в первичной профилактике сердечно-сосудистых осложнений вызывает много вопросов. Имеющиеся в настоящее время рекомендации противоречивы – от полного отрицания необходимости терапии антиагрегантами до указания конкретных групп больных, которым должно быть рекомендовано назначение АСК [4, 5]. В основе столь противоречивых положений – данные клинических исследований. Единственным антиагрегантом, назначе- ние которого обсуждается с целью первичной профилактики, является АСК. Применение АСК в первичной профилактике, как правило, приводит к снижению риска сердечно-сосудистых осложнений, однако положительный эффект действия в значительной степени нивелируется возрастающим риском кровотечений, прежде всего желудочно-кишечных. Таким образом, проблема состоит в необходимости отбора больных, у которых польза от назначения антитромботической терапии превосходила бы риски, связанные с кровотечениями. Для оценки риска осложнений обычно используются шкалы риска.
