Проблема запора в практике врача амбулаторного звена

Запор – это расстройство, от которого страдает большое количество людей и которое оказывает негативное влияние на качество жизни. Он является одной из распространенных причин обращения за медицинской помощью к врачу амбулаторного звена. Проблема запора зачастую может быть многофакторной, важными причинами которой являются низкое содержание клетчатки в пище, снижение количества потребления жидкости и малая физическая активность. У части пациентов запор не является хронической проблемой, а может возникать в определенный момент жизни, обычно длится ограниченное время и не является серьезной проблемой, решение которой может потребовать изменения питания и образа жизни либо кратковременного курса слабительных средств. В повседневной амбулаторной практике чаще встречается так называемый простой, или функциональный, запор. В то же время длительный, или хронический, запор зачастую является сложной проблемой как для врача, так и для пациента, нередко имеет вторичный характер, не только влияет на личное благополучие и качество жизни, но и является причиной немалых финансовых затрат, поскольку большая часть пациентов не сразу обращается за медицинской помощью, пытаясь заниматься самолечением, при этом длительно и не всегда успешно, переводя проблему в разряд хронической, усложняя в дальнейшем диагностический поиск и выбор тактики лечения. Хронический запор – это чаще проблема женщин и пожилых людей. Знание и понимание причин и возможностей диагностики запора, подходов к терапии, а также мер по его профилактике позволят врачу амбулаторного звена выбрать необходимую тактику ведения пациента. В статье представлен обзор литературы по этиопатогенетическим, диагностическим аспектам запора, а также возможности комплексного подхода к терапии. Удобными в практическом применении являются алгоритмы диагностического поиска и выбора терапии у пациента с запором, что также представлено в статье.
