Роль постнеоадъювантной терапии трастузумабом эмтанзином при HER2-положительном раке молочной железы

Аннотация

Широкое внедрение в практику анти-Her2 агентов изменило природное течение Her2-положительного рака молочной железы. Использование трастузумаба в неоадъювантных режимах, а в дальнейшем и двойной анти-Her2 блокады с пертузумабом, значимо повысило шансы на полное излечение. Однако среди больных с ранними и местнораспространенными формами Her2-положительного рака имеется когорта с крайне неблагоприятным прогнозом: не достигшие полной патоморфологической регрессии опухоли после проведенной неоадъювантной химиотерапии. Наличие резидуальной опухоли при Her2-положительном раке молочной железы долгое время оставалось лишь прогностически неблагоприятным фактором без возможности влияния на исход болезни. Результаты международного исследования III фазы KATHERINE, продемонстрировавшие высокую эффективность постнеоадъювантной терапии трастузумабом эмтанзином (T-DM1) в данной когорте пациентов, утвердили новый стандарт лечения. Благодаря адъювантной терапии T-DM1 возможность значимо улучшить отдаленные результаты определила предиктивные характеристики морфологического ответа в отношении выбора тактики лечения, что стало важным аргументом в пользу проведения неоадъювантной терапии у больных не только местнораспространенным, но и первично резектабельным Her2-положительным РМЖ с последующей персонализацией терапии.

В данной статье представлен собственный опыт проведения постнеоадъювантной терапии трастузумабом эмтанзином у молодой пациентки с  наличием резидуальной опухоли. Обобщены данные основных исследований при раннем Her2- положительном раке молочной железы.
