Клинический случай эозинофильного эзофагита у ребенка

Эозинофильный эзофагит – редкое хроническое воспалительное заболевание пищевода, внесенное в список орфанных и редких заболеваний МЗ РФ в 2024 г. В последние годы заболевание приобретает все большую значимость в клинической практике, особенно в педиатрии, т. к. его распространенность стремительно растет во всем мире. По данным литературы, эозинофильный эзофагит связан с аллергическими заболеваниями, такими как пищевая аллергия, атопический дерматит, бронхиальная астма и аллергический ринит, и представляет собой существенное клиническое и диагностическое затруднение из-за сходства симптомов с гастроэзофагеальной рефлюксной болезнью. Такие малоспецифичные симптомы, как срыгивания, тошнота и рвота, боли в животе во время еды наблюдаются у детей раннего возраста, жалобы на дисфагию отмечены у детей старшего возраста. Диагностика эозинофильного эзофагита – достаточно сложная задача из-за отсутствия специфических симптомов и маркеров, а также необходимости инвазивного подтверждения через эндоскопию и биопсию. Это приводит к частым случаям недооценки или запоздалой постановке диагноза, что, в свою очередь, может ухудшать прогноз и качество жизни пациентов. В статье приводятся клинические случаи эозинофильного эзофагита у детей младшего возраста с оценкой признаков, диагностическим поиском и принципами выбора терапии.
